Can a parent sign a form allowing their child (being a minor) to date an adult?

I would like to know if a parent can sign a form allowing a thirteen year old and a 18+ year old to date.

18+ and 13 year olds can date anyway. They just cannot have sex. There is no need for a form to allow them to date as they are allowed to anyway.
